            Had to go back to the title (aha, "Missing!") to be extra sure -- although hygienist should have been enough. Otherwise you would have been left wondering about a submission of Donna Karan. ;o) (Yes, I will call the dentist and make my overdue appointment this week.)

              Answer: TOOTH DECAY
              Explanation: I like that this one is a two-prong mechanism, in a way. First, six entries - relatively symmetrically - are missing a key component: TURNS (LOOSE), WHEAT (PASTE), PAINT (BRUSH), (SWEET) HEART, (PICKS) CLEAN, LIGHT (SABER). These six missing words can all precede or follow TOOTH in standard items and phrases (3-D came up during the fill, and that was fitting).
              But also, anchor entry 58-A seems clued in an odd way that would suggest something else is going on with the letters of the alphabet. Indeed, two letters are missing from this grid and all the clues as well: "D" and "K." (I had about 15 uses of "D" in the first edit of the clues, even when I thought I caught them all! Things like "and" and regular past tense had to be avoided). Phonetically, DK suggests DECAY, which also reinforces the six TOOTH-related words having a piece missing (decaying) from the grid. This finalizes to TOOTH DECAY, a fitting candy-related answer for an end-of-October meta! Brush and floss, folks!

                        Fun, but not in love with Puget being a fill. Before you know the TOOTH link you think PUGET SOUND works. I think the puzzle would have been better without that fill. Thanks Mikey!

                                    Very clever puzzle, Mikey! Found the missing words thing fairly quickly, except I had LICKS CLEAN as opposed to PICKS. Tried a bunch of different ways to make sense of it, but could not. Finally saw the tooth connection. Never saw the DK thing -- I guessed TOOTH FAIRY first (because who comes to collect your missing teeth?) and then finally DECAY. As luck would have it, I've got a dentist appointment next week, so good timing on this! Thanks.

                                      Clever. I totally lucked into DECAY; even with the nudges, that part of the puzzle wasn't sticking out to me. I really need to start using that program that I've seen mentioned, which shows how many times each letter has been used- that would have made the absence of D and K obvious!

                                              I got the TOOTH part right away. Without looking at the nudges I could think of only two possibilities -- FAIRY and DECAY. I couldn't find confirmation for either so I didn't submit. Now it turns out both were acceptable.
                                              Embarrassing not to get the second part since I used letter sounds in my own puzzle less than two months ago.
